Sub Code ng TCM System​​ 

This workbook is filled out and submitted to DHCS when an LGA wants to add Sub Codes to the LGA Profile on the TCM System.  Sub Codes are normally submitted with the Annual Participation Survey (APS).  The workbook is used when the number of Sub Codes requested exceeds the number of Sub Codes in APS.  The workbook is also used to submit additional sub codes through out the year.  The Sub Codes must be submitted by email to DHCS-TCM@dhcs.ca.gov from the LGA coordinator’s email account.​​
