The Justice-Involved Reentry Initiative works hand in hand with the following Medi-Cal initiatives to transform how care is delivered to members.
Providing Access and Transforming Health (PATH)
- PATH is a five-year, $1.85 billion initiative to build up the capacity and infrastructure of on-the-ground partners to successfully participate in the Medi-Cal delivery system and implement ECM, Community Supports, and Justice-Involved services.
- The PATH Justice-Involved (JI) Capacity Building Program provides funding to support the implementation of statewide justice-involved initiatives, including pre-release Medi-Cal enrollment and suspension processes and the delivery of Medi-Cal services in the 90 days prior to release.
- The PATH Capacity and Infrastructure Transition, Expansion and Development (CITED) initiative provides funding to enable the transition, expansion and development of Enhanced Care Management (ECM) and Community Supports capacity and infrastructure.

Enhanced Care Management and Community Supports
ECM is a new statewide Medi-Cal benefit available under the managed care delivery system to eligible members with complex needs. Individuals exiting incarceration and individuals who were incarcerated in the prior 12 months are eligible for ECM services.
ECM provides access to a single Lead Care Manager who provides comprehensive care management and coordinates their health and health-related care and services. ECM also connects members to the quality care they need, no matter where members seek care—at the doctor, the dentist, with a social worker, or at a community center. Community Supports are services that help address members’ health-related social needs, help them live healthier lives, and avoid higher, costlier levels of care. These include support to secure and maintain housing, access to medically tailored meals to support short term recovery, and a variety of other community-based services. Behavioral Health. The behavioral health components of CalAIM are designed to support whole-person, integrated care; move the administration of Medi-Cal behavioral health to a more consistent and seamless system by reducing complexity and increasing flexibility; and improve quality outcomes, reduce health disparities, and drive delivery system transformation and innovation through improvements to behavioral health policies and the launch of behavioral health payment reform. The majority of these policy changes launched in 2022, but implementation will continue through 2027. - The California Behavioral Health Community-Based Organized Networks of Equitable Care and Treatment (BH-CONNECT) Waiver webpage provides information about the Medicaid Section 1115 Demonstration application, submitted in 2023, to increase access to and improve mental health services for Medi-Cal members statewide.
