2024年1月1日、すべてのMedi-Calマネージドケアプラン(MCP)は、成人および小児亜急性期ケアのための施設長期ケア(LTC)サービスをカバーする責任を持つようになりました。​​   

MCPは、メンバーが亜急性期ケアサービスのプロバイダーや他のプロバイダーから必要とするケアを調整します。MCPは、メンバーに医療提供者のネットワークとケアコーディネーションサービスを提供し、メンバーのケアの管理を支援します。​​

Contracting with DHCS’ Subacute Care Unit​​  

MCPs must ensure that if a Medi-Cal member needs adult or pediatric subacute care services, they are placed in a health care facility that is under contract for subacute care with the Department of Health Care Services’ (DHCS) Subacute Contracting Unit (SCU) or is actively in the process of applying for a contract with DHCS’ SCU. A list of approved and active contracted facilities can be found on the DHCS SCU website.
