Program of All-Inclusive Care for the Elderly
Program of All-Inclusive Care for the Elderly (PACE) 케어 모델은 필요한 모든 예방, 1차, 급성 및 장기 케어 서비스를 제공하고 조정하는 PACE 센터에서 학제 간 팀 접근 방식을 사용하여 포괄적인 의료/사회 서비스 전달 시스템을 제공합니다. 이 서비스는 요양 시설에 거주할 수 있는 노인에게 제공됩니다. PACE 모델은 자격을 갖춘 개인이 가능한 한 오랫동안 자택에서 독립적으로 생활할 수 있도록 지원합니다. 자격을 갖추려면 55세 이상이고, PACE 서비스 지역에 거주하며, 보건의료서비스부에서 요양원 수준의 케어 자격이 있다고 판단하고, 등록 시점에 자택이나 지역사회에서 안전하게 생활할 수 있어야 합니다.
중요 공지 사항
Effective 12:00AM, November 20, 2025, the Department of Health Care Services (DHCS) will impose a PACE application pause for a minimum of two years.
During the two-year pause, DHCS will not accept PACE initial applications for entities wishing to become new PACE organizations or service area expansion applications from existing PACE organizations.
Please refer to the PACE Application Pause and Related Changes Policy Letter.
