Covered California - Single Streamlined Application

The Department of Health Care Services (DHCS) and Covered California have partnered to create a Single Streamlined Application for affordable health care coverage. You can use the Single Streamlined Application to apply for a range of affordable health care coverage options, including free or low cost Medi-Cal. The application process can be completed online, in-person, by phone, fax or mail.
The Single Streamlined Application is available in English and additional languages. Go to Covered California to complete an online application for Medi-Cal or low cost health insurance. Or you can print and fill out the Single Streamlined Application provided below in English and additional languages.
