The California 811 Project Rental Assistance (PRA) program is a collaborative formal partnership between the California Department of Health Care Services (DHCS), the State’s Medicaid agency, the California Department of Developmental Services (DDS) and the State’s three housing entities: the California Housing Finance Agency (CalHFA), the California Department of Housing and Community Development (HCD), and the California Tax Credit Allocation Committee (CTCAC).
California’s Project Rental Assistance Demonstration (Section 811) program to provide permanent supportive housing for extremely low-income persons with disabilities, particularly those who reside in a long-term care facility and desire to return to community living. A primary barrier to returning to the community from a long-term facility is lack of affordable housing.
Để đủ điều kiện tham gia Chương trình này, người nộp đơn phải đáp ứng tất cả các yêu cầu sau:
- Trong độ tuổi từ 18-61;
- Tàn tật;
- Sống tại cơ sở chăm sóc sức khỏe dài hạn trong ít nhất 60 ngày và mong muốn trở lại cuộc sống cộng đồng hoặc có nguy cơ bị đưa vào viện; và
- Những cá nhân đủ điều kiện hưởng Medi-Cal và được hưởng các dịch vụ và hỗ trợ tại cộng đồng.
