On December 29, 2022, the President signed the Consolidated Appropriations Act, 2023 (CAA) into law. The Act authorized an extension of the Money Follows the Person (MFP) Demonstration (known as California Community Transitions (CCT)) and appropriated additional funding for 2024 through 2027. In response, CCT transitions have been authorized to continue through December 31, 2027.​​ 

Chương trình chuyển đổi cộng đồng California​​ 

In January 2007, the California Department of Health Care Services (DHCS) was awarded special federal grant number 1LICMS300149 to implement a Money Follows the Person (MFP) Rebalancing Demonstration, known as “California Community Transitions” (CCT). 
DHCS works with designated CCT Lead Organizations (LOs) to implement the CCT program. Facility stays for short-term rehabilitation services reimbursed by Medicare are not counted toward the 60-day required period.  Individuals are eligible for this program if they have a physical and/or mental disability and have resided in a state-licensed health care facility for a period of 60 consecutive days or longer (e.g., with at least 1-day of Medi-Cal covered services). There is no age restriction for this program.
NOTE: Facility stays for short-term rehabilitation services reimbursed by Medicare are not counted toward the 60-day required period.​​  

Dịch vụ CCT LO​​ 

• Employ or contract with transition coordinators who work directly with willing and eligible individuals, support networks, and providers to facilitate and monitor beneficiaries’ transitions from facilities to the community settings of their choice.​​

State Funded California Community Transitions – Like  Program – Last Day to Enroll Participants Using this Criteria is December 31, 2025​​

The approval of AB 133 allows for the roll out of a state-funded, CCT–like program that had been rendered invalid because of a federal amendment to the Money Follows the Person (MFP) Demonstration authorizing statute.  AB 133 aligns state statute with the amended federal statute, by reducing the required period of residence in an inpatient facility from 90 to 60 days. The State-funded, CCT-like program allows CCT Lead Organizations to provide transition services to Medi-Cal beneficiaries who have not yet met the federal, MFP residency eligibility criteria, to help reduce the amount of time beneficiaries are required to remain in an institution during the COVID-19 public health emergency (PHE). 24 hours through 60 days in a medical facility with at least 1-day of Medi-Cal qualifies the individual for the State funded CCT.​​  

Dựa trên Bộ luật Phúc lợi và Viện dưỡng California (WIC) Mục 14196.2(c), bắt đầu từ ngày 1 tháng 1, 2026 Bộ Dịch vụ Chăm sóc Sức khỏe (DHCS) sẽ ngừng ghi danh những người hưởng lợi Medi-Cal trước đây đủ điều kiện tham gia Chương trình Tương tự CCT do Nhà nước Tài trợ. Given this policy change, the last date an LO should enroll Medi-Cal beneficiaries under the State-Funded CCT-Like Program requirements is December 31, 2025.

DHCS is  altering the process for reviewing new CCT LO applications. The department is receiving a large volume of applications from interested providers, and some areas of the state have more interest than others. To ensure that beneficiary care coordination needs are being met throughout the state, DHCS is prioritizing applications from providers interested in serving areas of the state that are currently underserved.​​

If you are interested in becoming an approved CCT LO, please submit an email with your interest to California.CommunityTransitions@dhcs.ca.gov. The email shall include but not limited to the following information:​​

• Cơ sở điều dưỡng (NF) Chuyển đổi/Chuyển hướng các cơ sở hỗ trợ sinh hoạt​​

• Một thành viên không thể nhận cả hai cùng một lúc.​​
