移动验光服务 (MOS) 计划是如何创建的？
2023 年 10 月，加州立法机构通过了参议院第 502 号法案 (SB 502)，旨在通过卫生保健服务部与合格的移动验光办公室之间的合作，让加州学校的低收入儿童更容易获得视力服务。流动验光办公室在学校环境中提供免费视力筛查、视力检查、眼镜和眼镜配戴服务。
如何成为 DHCS 的 MOS 提供商？
Providers must have a contractual agreement in place with DHCS before providing allowable vision services and submitting claims for costs to DHCS in this program. Interested parties must be Medi-Cal certified vision services providers. For a list of requirements to be a certified MOS provider, please refer to SB 502.
MOS 提供商的要求是什么？

我如何成为捐赠者？
Interested donors will need to contact a current DHCS MOS Provider to receive a Donor Guideline Questionnaire. Then you will submit the completed questionnaire to DHCS via email at MOSClinic@dhcs.ca.gov for review to ensure there are no financial conflicts of interest.
