人口健康管理
The California Department of Health Care Services (DHCS) is releasing Invitation for Proposal (IFP) #21-10375, for the Population Health Management (PHM) procurement. In response to California Assembly Bill 133 (AB 133) under Welfare and Institutions code 14184.204, which requires DHCS to develop, implement, and support a Population Health Management Service, DHCS is soliciting an IFP from qualified Contractors for this strategic initiative. In addition, this IFP will request Proposers to submit options to DHCS for implementation of enhanced population health management services that are not currently supported or utilized by DHCS.
At the discretion of DHCS, a single contract will be awarded to the most responsive and responsible PHM Service firm using scoring as a guide. This IFP is open to all eligible firms and/or individuals that meet the qualification requirements, including commercial businesses, non-profit organizations, State or public universities (including auxiliary organizations) and other entities.
DHCS conducted a Voluntary Pre-Proposal Web Conference on Monday, May 16, 2022, 10:00 a.m. to 11:30 a.m.

IMPORTANT: This IFP has officially released to the California State Contracts Register (CSCR) using the Cal eProcure/Fi$Cal website. Please use the instructions provided below to navigate to, and download, the IFP. All subsequent document releases for this IFP will also be made to the Cal eProcure/Fi$Cal website. As noted on the CD home page, it is highly recommended that all vendors wishing to contract with the State of California complete registration for the Cal eProcure/Fi$Cal system as it is the official location for releases. Registration will also allow vendors to receive automatic notifications when contracting opportunities are posted for business categories of their choosing. If selected as a contractor, this system will be used for payments covered by the contract.
