Money Follows the Person (MFP) Rebalancing Demonstration (稱為「California Community Transitions」(CCT)) 經由 Patient Protection and Affordable Care Act (Patient Protection and Affordable Care Act) 第 2403 節、Medicaid Extenders Act of 2019 (Medicaid Extenders Act of 2019)、Medicaid Services Investment and Accountability Act of 2019 (Medicaid Services Investment and Accountability Act of 2019)、Sustaining Excellence in Medicaid Act of 2019 (Medicaid Sustaining Act of 2019)、Further Consolidated Appropriations Act of 2020 (Consolidated Appropriations Act of 2020)、Coronavirus Aid, Relief, and Economic Security Act, 2020 (Coronavirus Aid, Relief, and Economic Security Act, 2020) 及 Consolidated Appropriations Act, 2021 (Consolidated Appropriations Act, 2021) 修訂。​​ 

2022 年 12 月，聯邦政府延長 MFP 補助金的期限，並撥出額外的資金來分配給州資助人。CCT 轉換服務目前在 2027 年開始提供。​​  

過渡到社區後，過渡協調員將對參與者進行 365 天的監督，稱為示範期。示範期允許參與者和過渡協調員對過渡後確定的需求做出回應，CCT 參與者居住在自己的家中、公寓或經批准的社區護理設施中，並獲得包含在其個人綜合服務計劃中的Long-Term Services and Supports 。 在示範期結束時，仍然有資格享受 Medi-Cal 的個人將繼續在他們居住的地方獲得 Medi-Cal 和其他居家和社區服務 (HCBS)。​​ 

州資助的加利福尼亞社區轉型 — 類似計劃​​ 

27 年 7 月 2021，《議會條例草案》（AB）133 章已成為法例。​​ 

AB 133 法案的批准使得一項由州政府資助的、類似 CCT 的Program得以推出，該項目此前因聯邦政府對“資金跟隨個人 (MFP) 示範項目授權法規”的修正案而失效。 AB 133 法案使州法律與修訂後的聯邦法律保持一致，將住院治療的必要期限從 90 天減少到 60 天。州政府資助的類似 CCT 的Program允許 CCT 牽頭組織向尚未達到聯邦 MFP 居住資格標準的Medi-Cal受益人提供過渡服務，以幫助減少受益人在 COVID-19 公共衛生緊急狀態 (PHE) 期間需要在機構中停留的時間。 在醫療機構接受治療 24 小時至 60 天，且至少有 1 天享有 Medi-Cal 醫療補助的個人，有資格獲得州政府資助的 CCT 補助。​​

9 月30 ， 2022 ， California福利和機構法典第 14196.6 條進行了修訂，將 CCT 類似Program的結束日期從 12 月31 ， 2023延長至 12 月31日， 2026 。​​
