Enhanced Care Management & Community Supports | Medi-Cal Transformation

​Enhanced Care Management (ECM) and Community Supports work hand in hand with the following Medi-Cal initiatives to transform how care is delivered to members.

Inc​entive Pa​​yment Program (IPP)

California is transforming Medi-Cal by offering new and improved services to ensure Californians get the care they need to lead healthier lives. The CalAIM IPP supports the implementation and expansion of Enhanced Care Management, Community Supports, and other CalAIM initiatives by providing incentives to Medi-Cal managed care plans that meet key implementation targets. ​

Providing Access and Tran​​​sforming Health (PATH)

Providing Access and Transforming Health (PATH) is a five-year $1.85 billion initiative to build up the capacity and infrastructure of on-the-ground partners, such as community-based organizations (CBOs), public hospitals, county agencies, and tribes, to successfully implement Enhanced Care Management, Community Supports, and Justice-Involved services. PATH funding helps these organizations participate in the Medi-Cal delivery system to ensure access to new health care services and benefits for members, and improve their health outcomes. By funding providers and CBOs statewide, with a particular focus on those serving communities that have been historically under-resourced and underserved, PATH will help California advance health equity. 

Population Health M​​anagement (PHM)

The PHM program is designed to proactively assess and address the care needs of members with tailored interventions. By 2023, plans are expected to be responsible for the care of more than 90 percent of Medi-Cal members. Establishing a unified, statewide approach to PHM ensures that all members — children, their parents, pregnant persons, adults, older adults, and people with disabilities — have access to a whole-system, person-centered program that leads to longer, healthier lives, improved clinical outcomes and a reduction in disparities. The launch of PHM is part of a broader arc of change to improve health outcomes across the state, which include the implementation of ECM and Community Supports. 

Justice-Invol​ved Initiative

Justice-involved individuals - people who are now, or have ever spent time, in jails, youth correctional facilities, or prisons - are at higher risk for injury and death than the general public. California became the first state in the nation approved to offer a targeted set of Medicaid services to youth and eligible adults in state prisons, county jails, and youth correctional facilities for up to 90 days prior to release. Through the Justice-Involved initiative, California w​ill offer a targeted set of Medicaid services to youth and eligible adults in state prisons, county jails, and youth correctional facilities for up to 90 days prior to release, as well as access to key services to help them successfully return to their communities.
