BH-CONNECT Incentive Program County-Specific Funding Allocations
Each county participating in the BH-CONNECT Incentive Program is eligible to earn up to a capped amount of funding each year based on a county-specific allocation. This document outlines county-specific allocations (total computable), including:
- Total Funding Allocation: The total funding the county is eligible to earn over the five-year Incentive Program (2025-2029). This figure is inclusive of the funding the county is eligible to earn for Submission 1, Submission 2, and all subsequent submissions.
- Submission 1 Funding Allocation: The funding the county is eligible to earn for Submission 1 (which was due June 30, 2025). Submission 1 is the Gap-Filling Plan measure for 2025 as indicated in Table 4 of the Incentive Program Scoring & Benchmarks Manual (23.8% of available 2025 funding).
- Submission 2 Funding Allocation: The funding the county is eligible to earn for Submission 2 (which is due June 30, 2026). Submission 2 is comprised of three measures for 2025: Data Sharing, Outreach and Crisis Services as indicated in Table 4 of the Incentive Program Scoring & Benchmarks Manual (14.4% of available 2025 funding).
Participating BHPs may receive Incentive Program funding for meeting specified performance targets in accordance with the BH-CONNECT Section 1115(a) Demonstration Special Terms and Conditions (STCs) and Incentive Program Protocol, Scoring & Benchmarks Manual, and Technical Specifications Manual.
