Dual Eligibles Coordinated Care Demonstration – Cal MediConnect​​ 

Cal MediConnect was a healthcare program designed to provide coordinated care for individuals who are eligible for both Medicare and Medi-Cal. It combined the benefits of both programs into one health plan, with additional care coordination benefits. On January 1, 2023, the companies that provided Cal MediConnect plans transitioned to offering Medicare Medi-Cal plans (Medi-Medi Plans or MMPs) that continue to coordinate healthcare delivery for this population.​​ 

La demostración fue una asociación entre California Medi-Cal el Programa de de y los Centros de Servicios de y Medicare Medicaid (CMS) y tenía como objetivo crear una experiencia de prestación de servicios sin interrupciones para los beneficiarios con doble elegibilidad, con los objetivos finales de una mejor coordinación de la atención, mejores resultados de salud y un sistema de prestación más eficiente.​​  

Cal MediConnect was a part of California’s larger Coordinated Care Initiative (CCI). Building on many years of stakeholder discussions, the CCI was enacted in July 2012 through SB 1008 (Chapter 33, Statutes of 2012) and SB 1036 (Chapter 45, Statutes of 2012). Further updates and clarifications to this initiative were enacted in June 2013 through SB 94 (Chapter 37, Statutes of 2013).​​ 

In 2014, the Cal MediConnect program was ultimately implemented in seven counties: Los Angeles, Orange, Riverside, San Bernardino, San Diego, San Mateo and Santa Clara. The transition to Medicare Medi-Cal plans in 2023 continues the goal of coordinated care for this population.
