이 페이지는 회원님과 같은 Medi-Cal 회원의 건강 정보 공유 규칙 이해를 돕기 위해 마련되었습니다. 데이터를 의사, 의료진 및 의료 보험과 공유하면 치료를 개선하고 적시에 지원을 받을 수 있습니다.​​

개인 정보 보호​​ 

Your health information is protected. Under the Health Insurance Portability and Accountability Act (HIPAA), you have the right to your health data. Sharing your health information with doctors, health plans, and your care team can result in better health outcomes, improve patient safety and coordinated care, and ensure timely access to your services. You can set some limits on who sees information about your health. However, most sharing of information for treatment, payment, and operational needs doesn’t need your consent.​​

When you see your provider for the first time, you might get a consent form. This form lets you pick what information is shared, with whom, for how long, and for what purpose. You can deny sharing information in certain circumstances or change who can see your information. If you change your mind about who you want to see your information, that’s okay. Just talk to your provider to learn more about this process.​​

Your health plan has your claims information and will grant you access to all explanation of benefits information. Your providers have your health records. If you want your records, ask your provider. You can also check out “Your Patient Privacy Rights: A Consumer Guide to Health Information Privacy in California” from the Office of the California Attorney General, and the Medi-Cal Notice of Privacy Practices.

  • AB 133은 2021년 7월에 통과된 법안으로, 캘리포니아 주법을 변경하여 CalAIM에 따라 서비스를 받는 사람들을 위한 데이터 교환 및 치료 조정을 촉진합니다(데이터 공유 승인 지침 2.0의 AB 133에 대해 설명하는 섹션 3 참조). 특히 AB 133은 주 개인정보 보호법이 서명된 동의 없이 데이터 공유를 허용하지 않는 상황에서도 데이터 공유를 허용합니다. 이는 사람들이 치료를 받을 수 있도록 돕기 위한 목적으로 이루어졌습니다.

CalAIM’s Population Health Management program helps members like you access a comprehensive set of services based on your needs and preferences, which leads to improved health outcomes and health equity.​​ 

인구 건강 관리 시행을 지원하기 위해 DHCS 에서는 Medi-Cal 회원을 위한 주 전역의 인구 건강 관리 서비스를 시작합니다. 인구 건강 관리 서비스는 광범위한 Medi-Cal 의료 팀에 과거 행정, 의료, 행동, 치과, 사회복지 서비스 데이터 및 현재 서로 다른 출처의 기타 프로그램 정보를 포함하여 Medi-Cal 가입자의 건강 기록, 요구 사항 및 위험에 대한 데이터 액세스 및 가용성을 제공할 것입니다.  인구 건강 관리 서비스는 데이터 정확성을 개선하고 인구 건강 동향과 다양한 인구 건강 관리 개입의 효과를 파악하고 감독을 강화하는 DHCS의 능력을 향상시킬 것입니다.​​

Medi-Cal Managed Care 플랜은 향상된 케어 관리(ECM)라는 새로운 혜택을 제공합니다. 이 혜택은 프로그램 자격이 있는 경우 임상 및 비임상 요구 사항을 해결해 줍니다. 이 프로그램에는 모든 건강 및 건강 관련 서비스를 조정하는 단일 리드 케어 매니저가 포함되어 있습니다.​​  

또한 사회적 필요를 충족하기 위해 커뮤니티 지원을 이용할 수도 있습니다. 커뮤니티 지원은 의료 맞춤형 식사 또는 주택 확보 및 유지 지원 등 회원님의 의료적 필요를 지원하고 건강을 유지하는 데 도움이 되는 서비스입니다. Medi-Cal Managed Care 플랜은 기존 의료 서비스나 환경에 대한 저렴한 대안으로 커뮤니티 지원 서비스를 제공합니다.​​
