In January 2007, the California Department of Health Care Services (DHCS) was awarded special federal grant number 1LICMS300149 to implement a Money Follows the Person (MFP) Rebalancing Demonstration, known as “California Community Transitions” (CCT). 
DHCS works with designated CCT Lead Organizations (LOs) to implement the CCT program. Facility stays for short-term rehabilitation services reimbursed by Medicare are not counted toward the 60-day required period.  Individuals are eligible for this program if they have a physical and/or mental disability and have resided in a state-licensed health care facility for a period of 60 consecutive days or longer (e.g., with at least 1-day of Medi-Cal covered services). There is no age restriction for this program.
NOTE: Facility stays for short-term rehabilitation services reimbursed by Medicare are not counted toward the 60-day required period.​​  

CCT LO 서비스​​ 

• Employ or contract with transition coordinators who work directly with willing and eligible individuals, support networks, and providers to facilitate and monitor beneficiaries’ transitions from facilities to the community settings of their choice.​​ 
• Conduct assessments to determine the participant’s level of care.​​

• 참가자가 가정과 지역사회에서 안전하게 지낼 수 있도록 여러 전달 시스템에서 참가자가 이용할 수 있는 서비스를 파악하고 조정하며, 필요에 따라 추가 지원 서비스를 소개합니다(필요에 따라).​​

State Funded California Community Transitions – Like  Program – Last Day to Enroll Participants Using this Criteria is December 31, 2025​​ 

0}년 2021월 27일, 하원 법안(AB) 133이 법으로 제정되었습니다.​​ 

AB 133의 승인으로 자금 추적(MFP) 시범 승인 법령의 연방 개정으로 인해 무효화되었던 주 정부 지원의 CCT와 유사한 프로그램을 시행할 수 있게 되었습니다.  AB 133은 입원 환자 시설의 필수 거주 기간을 90일에서 60일로 단축하여 주 법령을 개정된 연방 법령과 일치시킵니다. 주정부가 자금을 지원하는 CCT 유사 프로그램을 통해 CCT 선도 기관은 연방, MFP 거주 자격 기준을 아직 충족하지 못한 Medi-Cal 수혜자에게 전환 서비스를 제공하여 COVID-19 공중 보건 비상 상황(PHE) 동안 수혜자가 시설에 머무는 시간을 줄일 수 있도록 지원합니다. 의료 시설에서 24시간~60일 동안 최소 1일 이상 Medi-Cal을 받은 경우 주 정부 지원 CCT를 받을 수 있는 자격이 주어집니다.​​  

캘리포니아 복지 및 기관 코드(WIC) 섹션 14196.2(c)에 근거합니다, 1월부터 1, 2026 보건의료 서비스국(DHCS)은 이전에 주정부 지원 CCT 유사 프로그램에 자격이 있었던 Medi-Cal 수혜자의 등록을 중단합니다. 또한 1월 1일부터 1, 2027, DHCS는 주정부 지원 CCT 유사 프로그램에 대한 서비스 제공을 중단합니다.​​  

Given this policy change, the last date an LO should enroll Medi-Cal beneficiaries under the State-Funded CCT-Like Program requirements is December 31, 2025.

CCT LO 신청 절차​​ 

DHCS는 새로운 CCT LO 신청서를 검토하는 절차를 변경하고 있습니다. 해당 부서에서는 관심 있는 제공업체로부터 많은 양의 신청서를 받고 있으며, 일부 주에서는 다른 지역보다 더 많은 관심을 보이고 있습니다. 수혜자 치료 조정 요구가 주 전역에 걸쳐 충족되도록 보장하기 위해 DHCS는 현재 서비스가 부족한 주 내 지역에 서비스를 제공하는 데 관심이 있는 제공자의 신청을 우선적으로 처리하고 있습니다.​​ 

DHCS는 이번 개발로 인해 불편을 드려 죄송하며, 앞으로 귀사와 협력할 수 있기를 기대합니다.​​   

If you are interested in becoming an approved CCT LO, please submit an email with your interest to California.CommunityTransitions@dhcs.ca.gov. The email shall include but not limited to the following information:​​  

• CCT LO의 이름​​

• CCT LO가 서비스를 제공하려는 서비스 지역(도시 및 카운티)​​

Once the inquiry is reviewed and it is identified that a CCT LO is needed in a particular geographic area, the DHCS team will inform you of the next steps. All application questions can be sent to California.CommunityTransitions@dhcs.ca.gov.​​  

커뮤니티 지원​​ 

Community Supports are services that Medi-Cal managed care plans (MCPs) utilize to address Medi-Cal members ‘ health-related social needs, help them live healthier, and avoid higher, costlier levels of care.  These include:​​ 

• 요양 시설(NF)의 생활 보조 시설 전환/전환​​ 
• 지역사회 전환 서비스/요양 시설에서 가정으로 전환하기​​ 
• 기타 다양한 커뮤니티 기반 서비스​​ 

자격:​​ 

• 회원은 CCT 프로그램과 요양 시설 전환/전환 커뮤니티 지원을 모두 받을 수 있습니다.​​ 
• 회원은 두 가지를 동시에 받을 수 없습니다.​​  

CCT’s Relationship to Community Transition Services/Nursing Facility Transition to a Home:​​ 

• 회원은 CCT 프로그램과 커뮤니티 전환 커뮤니티 지원을 모두 받을 수 있지만 동시에 두 가지를 모두 받을 수는 없습니다.​​
