計劃認證
健康照護服務部 (DHCS) 擁有唯一權力對所有門診酒精或其他藥物治療計畫（也稱為物質使用障礙 (SUD) 治療計畫）進行認證和監督，這些計畫提供治療、康復、戒毒或藥物成癮治療 (MAT) 服務。經 DHCS 認證的 SUD 治療課程必須遵守DHCS 酒精和其他藥物項目認證（2025 年 2 月修訂版）中概述的要求；這些認證要求立即生效，並取代先前的版本。
在以下環境下運營的 SUD 處理設施不受 DHCS 認證，但如果符合酒精和其他計劃認證（2025 年 2 月修訂）中所述的要求，則可自願尋求認證：
- 成人酒精或藥物濫用康復或治療設施、受影響駕駛計劃以及由 DHCS 授權的麻醉治療計劃。
- 根據第 2 部第 1 章（從第 1200 條開始）獲國家公共衛生部發牌的診所。
- 根據第 2 部第 2 章（從第 1250 條開始）獲國家公共衛生部發牌的醫療設施。
- 根據第 2 分區第 3 章（由第 1500 條開始）獲國家社會服務部發牌的社區照顧設施。
- 根據第 2 分區第 3.01 章（由第 1568.01 條開始）獲國家社會服務部授權，為患有慢性、危及生命的疾病人提供的住宿照顧設施。
- 根據第 2 部第 3.2 章（由第 1569 條開始）獲國家社會服務部發牌的長者住宿照顧設施。
- 成人日間保健中心已獲得州社會服務部根據第 2 部分第 3.3 章（始於第 1570 節）頒發的許可證。
- 教育守則所定義的公立小學和中學。
- 縣監獄和州懲教機構，包括青少年司法設施。
醫療 服務 提供者如需取得認證的相關資訊，請閱讀初始認證申請書 （DHCS 6040 ）中的說明和程序。申請人必須填寫初次認證申請表，並提交申請表中規定的所有必要文件和費用。
認證提供者，尋求有關在現有認證門診計劃中添加或修改服務的信息，應閱讀證明修訂申請（DHCS 60 42）中包含的說明和程序。 申請人必須填妥證明修訂申請表格，並提交申請中所指明的所有必要文件和費用。
Certified providers shall submit to DHCS the Application for Certification Renewal (DHCS 6043), biennial fees, and all required information at least ninety (90) days prior to the expiration of certification. Failure to submit the Application for Certification Renewal form and fees for certification renewal at least 90 days prior to the expiration of certification shall result in automatic termination of the certification at the end of the two-year period.
