The 1115 “Bridge to Reform” Waiver (Waiver) renewal of November 2010 was intended to identify and test alternative health care delivery models for the California Children’s Services (CCS) program. A CCS Demonstration Project (DP) was pursued to test the efficacy of transitioning the CCS program from a fee-for-service based health care delivery model to an organized health care delivery model with a capitated payment. The CCS DP tests new approaches to improve the quality of care provided to children (up to 21 years of age) with special health care needs and focuses on identifying and solving roadblocks to access to care and ensuring families receive appropriate health care services for their child or youth.

CCS DP 的目標包括：

4 月1日， 2013聖馬刁健康計畫 (HPSM) 與聖馬刁縣衛生系統合作，成為豁免下第一個運作的 CCS DP 試點計畫。DHCS 選擇 HPSM 參與 CCS DP 試點項目，採用全風險 Medi-Cal 管理式醫療計劃模式，透過加強護理協調來改善該縣的 CCS 計劃。HPSM 為約 1,400 名符合 CCS 資格的客戶提供全面的醫療保健服務，並管理和協調「整個」兒童的全方位醫療保健服務，包括定期健康評估、免疫接種、與 CCS 資格醫療狀況無關的初級保健服務和專科醫療保健服務。
