About the dashboards

Medi-Cal Connect Dashboards

Medi-Cal Connect has eight interactive dashboards that provide population-level insights. The dashboards help plans understand critical population health needs and identify gaps in care. Plans can also use the dashboards to monitor outcomes, disparities, and quality of care for their members.

Bảng điều khiển BH-CONNECT

The Behavioral Health Community-Based Organized Networks of Equitable Care and Treatment (BH-CONNECT) Dashboard visualizes BH-CONNECT Incentive Program measures. This dashboard help plans identify members with unmet needs, support care coordination, and monitor the impact of evidence-based practices.

DHCS uses the BH-CONNECT Dashboard to calculate behavioral health plan (BHP) performance on Incentive Program measures that directly impact incentive payments.

Behavioral Health Transformation Dashboard

The Behavioral Health Transformation Dashboard visualizes program measures to support data-driven decision-making and collaboration between Medi-Cal managed care plans (MCP) and BHPs. This dashboard includes measures across critical domains, including access to care, follow-up care, quality, outcomes, housing stability, incarceration, and child welfare involvement.

DHCS uses the Behavioral Health Transformation Dashboard to monitor, interpret, and communicate progress, areas of needs, and outcomes for behavioral health programs across the state.

Care Management Dashboard

The Care Management Dashboard provides a snapshot of member demographics for Enhanced Care Management (ECM) and Community Supports, including enrollment trends. This information helps plans track program metrics and advance equitable access to benefits and services.

Condition Prevalence Dashboard

The Condition Prevalence Dashboard gives health plans information about the prevalence of behavioral and medical conditions among members. The dashboard can help MCPs and BHPs evaluate utilization of preventive services and design strategies, programs, and services to strengthen population health outcomes and reduce disparities.

Health Equity Dashboard

Health Equity Dashboard provides insights into social drivers of health to support population health initiatives and efforts to reduce disparities. This dashboard includes economic status and eligibility, geography, age, ethnicity, race, language, and gender and care utilization patterns.

Program Impact Dashboard

The Program Impact Dashboard provides potential eligibility, enrollment, and utilization of ECM and Community Supports as well as potential eligibility for the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC).

RSST Dashboard

The Risk Stratification, Segmentation, and Tiering (RSST) dashboard provides county-level risk data to help plans better understand risk across populations. Plans can use this information to design population-wide engagement and care strategies and more effectively manage individual client risk.

Learn more about RSST requirements for health plans.

Quality Measure Dashboard

The Quality Measure Dashboard tracks statewide and plan-specific performance on quality metrics, like the Centers for Medicare & Medicaid Services (CMS) Medicaid Core Set, BH-CONNECT Incentive Program, and the DHCS Medi-Cal Accountability Set (MCAS) and Behavioral Health Accountability Set (BHAS). The dashboard provides visibility into DHCS’ calculation of rates, as well as opportunities for improved plan performance in the Medi-Cal program.
