In January 2007, the California Department of Health Care Services (DHCS) was awarded special federal grant number 1LICMS300149 to implement a Money Follows the Person (MFP) Rebalancing Demonstration, known as “California Community Transitions” (CCT). 
DHCS works with designated CCT Lead Organizations (LOs) to implement the CCT program. Facility stays for short-term rehabilitation services reimbursed by Medicare are not counted toward the 60-day required period.  Individuals are eligible for this program if they have a physical and/or mental disability and have resided in a state-licensed health care facility for a period of 60 consecutive days or longer (e.g., with at least 1-day of Medi-Cal covered services). There is no age restriction for this program.
NOTE: Facility stays for short-term rehabilitation services reimbursed by Medicare are not counted toward the 60-day required period.​​  

CCT LO 服务​​ 

  • Employ or contract with transition coordinators who work directly with willing and eligible individuals, support networks, and providers to facilitate and monitor beneficiaries’ transitions from facilities to the community settings of their choice.​​ 
  • Conduct assessments to determine the participant’s level of care.​​ 
  • 实施以人为本的支持服务。​​  
  • 努力确定和协调来自多个交付系统可供参与者使用的服务,目的是确保参与者在其家中和社区中的安全,并提供额外支持服务的推荐(根据需要)。​​

State Funded California Community Transitions – Like  Program – Last Day to Enroll Participants Using this Criteria is December 31, 2025​​ 

年 7 月27日, 2021议会法案 (AB) 133 被正式纳入法律。​​ 

AB 133 的批准使得一项由州政府资助的、类似 CCT 的计划得以推出,该计划之前曾因联邦对“钱随人走(MFP)示范授权法规”的修订而失效。AB 133 将州法规与修订后的联邦法规相一致,将住院时间要求从 90 天缩短至 60 天。该州资助的类似 CCT 的计划允许 CCT 牵头组织向尚未满足联邦 MFP 居住资格标准的 Medi-Cal 受益人提供过渡服务,以帮助减少受益人在 COVID-19 公共卫生紧急情况 (PHE) 期间必须留在机构的时间。在医疗机构中度过 24 小时至 60 天并且至少享受 1 天 Medi-Cal 福利的个人有资格获得州政府资助的 CCT。​​  

根据加州福利和机构法典 (WIC) 第14196.2(c)条,自1 1 月起, 2026卫生保健服务部 (DHCS) 将停止招收之前有资格参加州资助的 CCT 类计划的 Medi-Cal 受益人。此外,从1年2027月开始,DHCS 将停止为国家资助的 CCT 类计划提供服务。​​  

Given this policy change, the last date an LO should enroll Medi-Cal beneficiaries under the State-Funded CCT-Like Program requirements is December 31, 2025.

CCT LO 申请流程​​

If you are interested in becoming an approved CCT LO, please submit an email with your interest to California.CommunityTransitions@dhcs.ca.gov. The email shall include but not limited to the following information:​​  

  • CCT LO 的姓名​​ 
  • CCT LO 的位置(城市和县)​​ 
  • CCT LO 计划服务的覆盖区域(城市和县)​​ 
  • 联络人​​ 

Once the inquiry is reviewed and it is identified that a CCT LO is needed in a particular geographic area, the DHCS team will inform you of the next steps. All application questions can be sent to California.CommunityTransitions@dhcs.ca.gov.​​  

社区支持​​ 

Community Supports are services that Medi-Cal managed care plans (MCPs) utilize to address Medi-Cal members ‘ health-related social needs, help them live healthier, and avoid higher, costlier levels of care.  These include:​​ 

  • 护理机构 (NF) 辅助生活设施的转型/转移​​ 
  • 社区过渡服务/护理机构过渡到家庭​​ 
  • 各种其他社区服务​​ 

合格:​​ 

  • 会员可以同时符合 CCT 计划和护理机构过渡/转移社区支持的资格。​​ 
  • 会员不能同时获得这两项福利。​​  

CCT’s Relationship to Community Transition Services/Nursing Facility Transition to a Home:​​ 

  • 会员可以同时符合 CCT 计划和社区过渡社区支持的资格,但不能同时获得这两项计划。​​
