In January 2007, the California Department of Health Care Services (DHCS) was awarded special federal grant number 1LICMS300149 to implement a Money Follows the Person (MFP) Rebalancing Demonstration, known as “California Community Transitions” (CCT).
DHCS works with designated CCT Lead Organizations (LOs) to implement the CCT program. Individuals are eligible for this program if they have a physical and/or mental disability and have resided in a state-licensed health care facility for a period of 60 consecutive days or longer (e.g., with at least 1-day of Medi-Cal covered services). There is no age restriction for this program.

State Funded California Community Transitions – Like Program – Last Day to Enroll Participants Using this Criteria is December 31, 2025

年 7 月27日， 2021议会法案 (AB) 133 被正式纳入法律。

The approval of AB 133 allows for the roll out of a state-funded, CCT–like program that had been rendered invalid because of a federal amendment to the Money Follows the Person (MFP) Demonstration authorizing statute. AB 133 aligns state statute with the amended federal statute, by reducing the required period of residence in an inpatient facility from 90 to 60 days. The State-funded, CCT-like program allows CCT Lead Organizations to provide transition services to Medi-Cal beneficiaries who have not yet met the federal, MFP residency eligibility criteria, to help reduce the amount of time beneficiaries are required to remain in an institution during the COVID-19 public health emergency (PHE). 24 hours through 60 days in a medical facility with at least 1-day of Medi-Cal qualifies the individual for the State funded CCT.

Given this policy change, the last date an LO should have enrolled Medi-Cal beneficiaries under the State-Funded CCT-Like Program requirements is December 31, 2025.

CCT LO 申请流程

DHCS is altering the process for reviewing new CCT LO applications. The department is receiving a large volume of applications from interested providers, and some areas of the state have more interest than others. To ensure that beneficiary care coordination needs are being met throughout the state, DHCS is prioritizing applications from providers interested in serving areas of the state that are currently underserved.
