老年人全包护理计划
老年人全包式护理计划 (PACE) 护理模式采用跨学科团队方法，在 PACE 中心提供和协调所有必要的预防、初级、急性和长期护理服务，提供全面的医疗/社会服务交付系统。服务对象是原本住在护理机构的老年人。PACE 模式让符合条件的个人尽可能长时间地保持独立并在家中生活。要符合条件，个人必须年满 55 岁、居住在 PACE 服务区、由卫生保健服务部确定符合疗养院护理水平，并且在注册时能够安全地住在家中或社区中。

Effective 12:00AM, November 20, 2025, the Department of Health Care Services (DHCS) will impose a PACE application pause for a minimum of two years.
During the two-year pause, DHCS will not accept PACE initial applications for entities wishing to become new PACE organizations or service area expansion applications from existing PACE organizations.
