CalAIM Concept Paper​​

DHCS is hosting a 30-day public comment period from July 23 to August 22, 2025, to solicit feedback on the Continuing the Transformation of Medi-Cal Concept Paper. This concept paper focuses on California’s Medi-Cal transformation efforts to date and highlights DHCS’ vision and goals for the five years, starting in 2027, after key federal waivers that authorize California Advancing and Innovating Medi-Cal (CalAIM) expire in December 2026.​​

Inilabas ng DHCS ang isang binagong panukala ng CalAIM noong Enero 8, 2021:​​

Pormal na inilabas ng DHCS ang panukala ng CalAIM noong Oktubre 29, 2019, sa mga pulong ng Stakeholder Advisory Committee (SAC) at Behavioral Health Stakeholder Advisory Committee (BH-SAC). Sa pagitan ng Nobyembre 2019 at Pebrero 2020, ang DHCS ay nagsagawa ng malawakang pakikipag-ugnayan sa stakeholder para sa parehong CalAIM at ang pag-renew ng mga pederal na awtoridad kung saan nagpapatakbo ang Medi-Cal (ibig sabihin, 1115 at 1915b waiver).​​

In 2019, DHCS established topic-specific stakeholder workgroups to explore specific sections of the CalAIM proposal. Pinamamahalaang Pangmatagalang Serbisyo at Mga Suporta at Duals Integration Workgroup​​ 

Sa konsultasyon sa mga pangunahing stakeholder, at sa pakikipag-ugnayan sa Master Plan ng Gobernador para sa Pagtanda, inilunsad ng DHCS ang Managed Long-Term Services and Supports (MLTSS) at Duals Integration workgroup noong Pebrero 2021. Ang layunin ng workgroup ay makipagtulungan sa mga stakeholder sa paglipat ng Cal MediConnect (CMC) sa isang statewide MLTSS at Dual Eligible Special Needs Plan (D-SNP) aligned enrollment structure. Ang patakarang ito ay naglalayong tumulong na matugunan ang mga layunin sa buong estado ng pagpapabuti ng pagsasama ng pangangalaga at pangangalagang nakasentro sa tao, sa ilalim ng parehong CalAIM at ng California Master Plan for Aging.​​

Grupo ng Pagpapayo ng Population Health Management (PHM).​​  

Launched in 2022, the PHM Advisory Group is comprised of cross-sector stakeholders who provide feedback and recommendations on many aspects of the CalAIM PHM program and the PHM Service. Members include a diverse set of leaders from across the state with representation from health plans, providers, counties, state departments, consumer organizations, and other groups. Specific topics for discussion will vary by meeting and will include screening and assessments, risk stratification and tiering, basic population health management, including wellness and prevention, care management, and patient engagement. While the PHM Advisory Group will be open to the public, active participation will be limited to Advisory Group members. The group will meet periodically through 2022.​​

Medi-Cal Strategy to Support Health and Opportunity for Children and Family​​
