Expanding Access to Integrated Care for Dual Eligible Californians

Under the California Advancing and Innovating Medi-Cal Initiative (CalAIM), the California Department of Health Care Services (DHCS) will transition Cal MediConnect (CMC) and the Coordinated Care Initiative (CCI) to a statewide Managed Long-Term Services and Supports (MLTSS) and Dual Eligible Special Needs Plans (D-SNP) structure.  Starting in 2023, Medicare Medi-Cal Plans (MMPs or Medi-Medi Plans) will provide integrated Medicare and Medi-Cal benefits to dually eligible beneficiaries. Members of Cal MediConnect plans will automatically transition to MMPs on January 1, 2023, as part of the CalAIM Initiative. This policy is intended to help meet the statewide goals of improving care integration and person-centered care, under both CalAIM and the California Master Plan for Aging (MPA).​

​CalAIM MLTSS and Duals Integration Workgroup

The CalAIM MLTSS and Duals Integration Workgroup will serve as a stakeholder collaboration hub for this effort, including health plans, stakeholders, and the U.S. Centers for Medicare & Medicaid Services (CMS).  This Workgroup will provide an opportunity for stakeholders to provide feedback and share information about policy, operations, and strategy for the Department's transition plan for dual eligibles and the CCI transition within the CalAIM initiative.

Initial workgroup discussions focused on policies and operations to transition Cal MediConnect members to Medicare Medi-Cal Plans. Future topics will be developed based on CalAIM goals and MPA topics around integrated care for dual eligible Californians.
