Medi-Cal Managed Care Boilerplate Contracts
The following is the current managed care contract boilerplate for all models of managed care beginning CY 2024, including County Organized Health Systems, Geographic Managed Care, Regional, Single Plan, and Two-Plan models. *Please note: All Managed Care Plan contracts have been formally amended to extend the term through December 31, 2025.

This webpage and its linked documents provide information of public interest regarding the Department of Health Care Services' (DHCS) executed amendments to Medi-Cal Managed Care Boilerplate Contracts (Amendments). These documents include both Amendments which have been reviewed and approved by the federal Centers for Medicare and Medicaid Services (CMS) as well as Amendments which are currently pending review and approval by the CMS. DHCS is posting the Amendments for the sole purpose of informing stakeholders and the public of changes to the Medi-Cal Managed Care Contracts. If CMS approval is not obtained, the documents linked on this webpage create no obligation on DHCS nor Managed Care Plans to comply with the provisions of the Amendments that CMS did not approve.
