Population Health Management Technical Assistance
The Department of Health Care Services (DHCS) is releasing this Request for Information (RFI) #24-069 to gather information from entities regarding technical assistance (TA) to support a policy program under the Population Health Management Program (PHM). In January 2024, DHCS, in collaboration with the California Department of Public Health (CDPH), provided guidance in the PHM Policy Guide requiring Mange Care Plans MCPs participation in the Community Health Assessments (CHAs)/and Community Health Improvement Plans (CHIPs) conducted by Local Health Jurisdictions (LHJs) as outlined in All Plan Letter (APL) 23-021.
The purpose of the TA is to ensure effective implementation of this policy by supporting and strengthening engagement among MCPs, LHJs and community stakeholders over time. Bridging gaps in understanding to align interpretations of policy expectations and enhance collaboration. Fostering a deeper understanding among MCPs of the health and social needs of members and the communities in which they live through cross-sector partnerships. Enhancing MCPs' ability to effectively engage in CHA/CHIP processes and develop high-impact, collaborative strategies to address the needs identified in the CHA and prioritized in the CHIP and reducing siloed approaches to population health management, fostering more integrated and effective strategies to improve the health and well-being members.
This RFI is issued for information and planning purposes only and does not constitute a solicitation. A response to this RFI is not an offer and cannot be accepted by the State to form a binding contract. Respondents are solely responsible for all expenses associated with responding to this RFI. DHCS asks willing Respondents to share nonbinding budgetary pricing information for each identified solution where requested. Pricing is only for planning purposes. Any pricing provided in a response to this RFI will not be considered an offer on the part of a Respondent.
