​​Child Health and Disability Prevention (CHDP) Program Transition Plan

​Per Senate Bill (SB) 184, the Department of Health Care Services (DHCS) authorizes to transition the Child Health and Disability Prevention Program (CHDP) by July 1, 2024. DHCS has conducted a stakeholder engagement process to inform the development of a transition plan, detailing milestones guiding the transition of the CHDP program to other existing Medi-Cal delivery systems. The CHDP Transition Plan includes the following key components: 

  • Children’s Presumptive Eligibility;
  • Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services;
  • Health Care Program for Children in Foster Care (HCPCFC);
  • CHDP-Childhood Lead Poisoning Prevention Program (CHDP-CLPP); and
  • Newborn Hearing Screening Program (NHSP)​
