Child Health and Disability Prevention (CHDP) Program Transition Plan
Per Senate Bill (SB) 184, the Department of Health Care Services (DHCS) authorizes to transition the Child Health and Disability Prevention Program (CHDP) by July 1, 2024. DHCS has conducted a stakeholder engagement process to inform the development of a transition plan, detailing milestones guiding the transition of the CHDP program to other existing Medi-Cal delivery systems. The
CHDP Transition Plan includes the following key components:
- Children’s Presumptive Eligibility;
- Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services;
- Health Care Program for Children in Foster Care (HCPCFC);
- CHDP-Childhood Lead Poisoning Prevention Program (CHDP-CLPP); and
- Newborn Hearing Screening Program (NHSP)
