California is transforming Medi-Cal through several initiatives to continue to drive quality of care improvements, streamline and reduce complexity, and build upon equity-focused, data-driven, and whole-person care. The Subacute Care Carve-In to Medi-Cal managed care standardizes adult and pediatric subacute care statewide in order to advance a more consistent and integrated system of managed care and increase access to comprehensive care coordination, care management, and a broad array of services for Medi-Cal members receiving subacute care.

What the Carve-In means for Californians

On January 1, 2024, all Medi-Cal managed care plans (MCPs) became responsible for covering institutional long-term care (LTC) services for adult and pediatric subacute care.  

MCPs coordinate the care members require from providers of subacute care services as well as from other providers. MCPs provide members with a network of health care providers and care coordination services to help manage member care.

Contracting with DHCS' Subacute Care Unit 
MCPs must ensure that if a Medi-Cal member needs adult or pediatric subacute care services, they are placed in a health care facility that is under contract for subacute care with the Department of Health Care Services' (DHCS) Subacute Contracting Unit (SCU) or is actively in the process of applying for a contract with DHCS' SCU. A list of approved and active contracted facilities can be found on the DHCS SCU website.
