Preadmission Screening and Resident Review (PASRR) is a federally mandated program, applicable to all Medicaid certified nursing facility applicants. The program determines if individuals with a serious mental illness (SMI), intellectual disability, developmental disability, or related conditions (ID/DD/RC) require nursing facility services, considering the least restrictive setting or any specialized services.

File Exchange (Hospitals)
Prior to initiating the file exchange, the hospital must confirm the PASRR process is completed with the correct resolution and print all PASRR documents for their medical records. Once the hospital sends the case to the Skilled Nursing Facility (SNF), they no longer have access to it. The file exchange must be initiated before the individual discharges to the SNF. Both Approvers and Users can initiate the file exchange. File Exchange instructions are located here: file exchange tutorial video.

File Exchanges for SNFs
To access the case and corresponding letters in the PASRR system, a SNF must accept the file exchange from the hospital. Only Approvers can accept the file exchange.
Reminder: The file exchange has a timer of 5 days from the day the file exchange was sent. Please have an approver check for incoming file exchanges on the dashboard at least once a day. File exchanges that aren't accepted within 5 days will remain with the original facility. The SNF should reach out to the discharging facility to resend the file exchange if the acceptance window was missed. If there are complications sending or retrieving a file exchange, please email ITServiceDesk@dhcs.ca.gov and provide the Case Identification Number (CID number) in question.
Multi-Factor Authentication (MFA) Reset
Account users have a two-step verification/MFA to access the PASRR system, in most cases. If the account holder loses access to the phone number associated with their account due to a new phone or new phone number, they may need to reset their MFA to log into the PASRR system. To request an MFA reset, please email ITServiceDesk@dhcs.ca.gov and request an “MFA Reset". Once the request has been completed by the PASRR team, we will email the point of contact with the following instructions:
Your Multi-Factor Authentication (MFA) was reset. Please log in here: https://portal.dhcs.ca.gov. You should be prompted to reselect the MFA. We recommend selecting the “I want to set up a different method" link (under the QR code) to use a personal cell phone and receive a code by text message option if your facility allows it. If not, you will need to use the Microsoft Authenticator App or choose a land line number and receive a code by call option.
Treatment Authorization Requests (TARS)
When submitting TARS for reimbursement, SNFs must include the PASRR documentation which can be downloaded from the PASRR system. If a resident review (RR) is submitted, please include all pages of the original preadmission screening (PAS) and the RR (all pages) together.
SNFs must include the Level I Screening and one of the following letters with their TAR submission:
- Exempted Hospital Discharge Letter
- Notice of No Need Letter
- LI - Categorical Letter
- LII – Categorical Letter
- LII – Attempt Letter - No SMI
- Determination Letter. Please ensure the recommended Level of Care (LOC) on the Determination Letter matches the admitting facility's LOC. If not, SNFs may submit a Reconsideration request in the PASRR system within 90 days.
