The Breast and Cervical Cancer Treatment Program (BCCTP) provides cancer treatment benefits to eligible low-income California residents diagnosed with breast and/or cervical cancer. Enrolling Providers play an imp​​​ortant role assisting individuals in obtaining the treatment and services they need by screening and enrolling them online.

​​Become a Medi-Cal Provider today!

​​To become a Medi-Cal provider, and to help individuals enroll into the BCCTP​ you can apply through the Provider Application and Validation for Enrollment (PAVE) System Webpage. ​​​

​Medi-Cal Providers interested in submitting applications to BCCTP

Only Medi-Cal providers may submit applications on behalf of applicants for a BCCTP eligibility determination. To do so, you also must participate in the Every Woman Counts (EWC) or Family Planning, Access, Care and Treatment (Family PACT) ​programs.

​To obtain a National Provider Identifier (NPI) User ID
​​Provider Enrollment at (916) 323-1945 for enrollment requirements


The Department of Health Care Services provides information on topics such as, eligibility, claim forms and claim submission, electronic billing, general resources, outreach, education and provider training, and frequently asked questions (FAQs), for newly enrolled Medi-Cal providers. If an individual would like to designate a person or organization to assist them with the application process, the Authorized Representative forms can be found below. Forms must be completed and signed by the applicant and designated Authorized Representative. 

  • MC 382​ - Appointment of Authorized Representative
  • MC 383 ​- Authorized Representative Standard Agreement for Organizations ​

BCCTP Qualifying Diagnoses​​

If the applicant's diagnosis is not on the list of Qualifying Diagnoses or is not otherwise specified (NOS), the current pathology report must be faxed or e-mailed, with any supporting medical records to (916) 440-5693; or BCCTP@dhcs.ca.gov.​ A DHCS Medical Consultant will review documentation to determine if the applicant has a qualifying diagnosis and needs treatment. BCCTP will inform you if the applicant can be enrolled.
