How do I become a donor?

The
MOS program relies entirely on private donations and federal funds to serve
low-income students throughout California. Without the support of donors, the
benefits of the program will be limited and children may not receive the care
they need to achieve better long-term life outcomes.
Potential donors will
need to contact a current DHCS MOS Provider to receive a Donor
Guideline Questionnaire. A list of current providers can be found on the Mobile Optometric Services Program Homepage. The questionnaire asks donors to
provide information about themselves or their organization to ensure the donor
does not have a financial conflict of interest related to the vision services
and/or vision related goods provided through the MOS Program. Once completed,
the questionnaire must be submitted to DHCS via email at MOSClinic@dhcs.ca.gov for review. Upon approval, DHCS will provide
further instruction on the donation process.
